 1. 工廠營業執照及稅務登記,及新的年審登記記錄 
 Business license and Tax Registration，with annual renewal
 2. 工廠的組織架構圖 
 Factory Organizational Chart
 3. 全廠員工花名冊 （包括姓名，工號，身份證號碼，入職日期，部門，社保購買情況等）
  All workers’ name list (including name, ID card#, Working ID #, entrance date, social insurance if applicable, etc.)
 4. 員工手冊/廠規廠紀
 Employee Handbook/Factory Regulations
 5. 工資與工作時間記錄(近兩年)，工資單樣版 
 Payroll & Attendance Records (past two years) & Pay slip sample
 6. 政府新文件. 如:低工資, 社會保險的規定, 綜合計時審批記錄等 
 Updated official notification or approval from local , such as minimum wage, social insurance andovertime approval or similar documentation.
 7. 招工廣告,相關招工程序描述以及內部人員需求表
 Example of hiring advertisement, and description of related recruitment procedure, headcount request form
 8. 員工人事檔案：新員工入職申請表, 員工身份證復印件及其他相關文件, 各種請假單, 獎懲及升遷記錄, 辭工記錄, 培訓記錄等
 Workers’ personnel files: Pre-employment application form, ID card Copy and related supporting files, all leave records, warning or disciplinary records, promotion records, resignation or termination records, training records.

 9. 社會保險及工傷保險花名冊, 繳費單據(原件)
  Social insurance record and work-related injury insurance, and thepayment record (original copy)
 10. 當地社會保險局的豁免聲明或者符合性證明(如有) 
Waiver from local labor bureau on social insurance (if applicable)
 11. 新的未成年工名單/當地政府備案相關資料/體檢報告等 
 Updated Young Workers’ name list/registration files to local labor bureau/health checkrecords, etc.
 12. 員工辭職報告(近一年) 
 Workers’ resignation records (past 12 months)
 13. 員工申訴記錄(近一年) 
 Workers’ grievance records (past 12 months)
 14. 集體勞動合同, 工會或其他工人組織記錄
 Collective bargaining agreement, activity reports/records and meeting minutes of TradeUnion or other worker committee.
 15. 近三個月銀行轉賬員工工資記錄及工資帳目
 Bank voucher of workers’ wage for recent three months and accounting record
 16. 全廠工人年度體檢報告(包括相關職業健康體檢記錄)
 All workers’ annual health check records (including related occupational health check records).
 17. 工人自愿加班記錄/文件 
 Workers’ Voluntary OT records/files.
 18. 工傷記錄(近一年) 
 Work-related injury records (past 12 months)
 19. 全廠員工相關培訓記錄(包括但不限于: 新員工培訓, 消防設備使用培訓, 化學品使用培訓, 個人防護用品PPE培訓, 機器安全操作培訓, 疏散演習, 工資工時培訓, 廠規廠紀培訓等) 
 Related training records (including orientation training, fire equipments operation training, chemical handling training, PPE training, machinery safety training, fire drills, wage and working hours training, factory regulations training, etc.).
